Bring a copy of your short forms for your appointment if you have not already filled them out online (or arrive 20 minutes early to fill out). Bring your driver’s license or ID and an insurance card/ information if you have any insurance that may help support you in your care.

At GLFC, we set aside special appointment times during the day to accommodate new patients.
This ensures we have enough time to know exactly what is going on with you and can work through making the best recommendations and providing the best individualized care.
