Bring a copy of your short forms for your appointment if you have not already filled them out online (or arrive 20 minutes early to fill out).

If you have not already filled out forms, make sure to arrive 30 minutes early so we can assist you in getting them completed.

At GLFC, we set aside special appointment times during the day to accommodate new patients.

This ensures we have enough time to know exactly what is going on with you and can work through making the best recommendations and providing the best individualized care.

We will always do our best to get you set up for an appointment promptly. If we can accommodate you at that time, we are happy to do so!
